Office Hours and Location:
Monday- Friday 8:00 AM- 5:00 PM
Saturday 8:00 AM- 12:00 noon
322 Beard Creek Rd., Edwards
Telephone: (970) 569-7520

Eligibility for clinic services:

  • Eagle Care Medical Clinic serves patients that are uninsured or who have Medicaid.
  • Patients must live in Eagle County.
  • Patients that carry insurance may be eligible for Eagle Care if their policy is for catastrophic illness or injury only.  Patients requesting care due to being underinsured must bring in documentation of their insurance plan for review.

Emergency Care:

  • The Eagle Care Medical Clinic is NOT an emergency care facility.  In case of an emergency please call 911 and/or report to an emergency room.

Appointments:

  • All patients are encouraged to call ahead for an appointment.
  • It is highly recommended to call in for an appointment as walk-in slots are quickly taken.
  • Saturdays are reserved for same day call-in care.
  • There currently are more patients than the clinic has capacity to serve.  Please be patient as clinic staff tries to provide the highest quality of care to all that need it.

Registration:

  • All patients must check in at the front desk.
  • At each visit patients will be required to sign a consent form and update contact information.
  • Each patient is required to complete and sign an income statement.  Patient’s income does not impact eligibility for care.  However, the clinic can access state and federal funds if we can collect income information on each patient.  Thus, it is important for the clinic to collect income information in order to keep our fees low.

Patient Safety and Infection Control:

  • During the winter months the lobby is divided in order to keep contagious illnesses from spreading.  The area next to the T.V. set is designated for those that are healthy.  The area next to the check- in desk is designated for those that have contagious illnesses.  Masks and hand gel are available to all patients and are located throughout the lobby area.
  • Each patient will receive a patient safety brochure at their initial visit.  Please review this brochure carefully.  Please advocate for your own health and safety.
  • All patients will need to evacuate if there is a fire alarm.  Clinic staff will assist patients to exit the clinic.  Please go to the parking lot to the east of the building near the circle drive and wait for the all clear signal before returning to the clinic.

Fees:

  • The clinic fees are based on Medicaid rates and are about 30% of usual and customary charges.  The Vail Valley Medical Center generously subsidizes most of the cost for a clinic visit.
  • Certain services such as labs, x-rays and imaging are provided through clinic referrals.  The fees for these services are based on hospital rates.
  • A sliding fee scale based on family income and size is available.  Contact 970-479-5133 for more information.

Payment:

  • Patients are asked to pay for the clinic visit at the time of service.  If unable to pay at the time of service, a bill will be sent.  Payment plans can be arranged.
  • Ancillary services such as labs, x-rays and imaging will be billed to your address.

Special Programs and Partnerships:

  • Uninsured women over the age of 40 who have a family income less than 250% of poverty may qualify for the Colorado Women’s Cancer Control Initiative and receive certain types of women’s health services for free.  Ask your provider for more information.
  • All established Eagle Care patients may receive a free HIV test upon request.
  • Many Eagle Care prenatal patients qualify for no-cost prenatal care through an Eagle County partnership.  Pregnant women should inquire at 970-748-3280.

Security:

  • The Eagle Care Medical Clinic is not responsible for loss, theft or damage to guests’ personal property.  Please keep your belongings with you.

Patient Satisfaction:

  • The goal for the Eagle Care Medical Clinic is to provide high quality care to all patients.  Your feedback is important.  Patients will receive a satisfaction survey in the mail after their visit.  Please complete this survey so we can improve our care.
